Thank you for giving Lake County Family Practice the opportunity to care for you and your family. Our physicians and staff believe that caring for you and your entire family will enable us to better understand you as a person. We will listen to what you have to say and make decisions with you about your health care needs. Our goals are to build trust and honesty, create open communications between physician and patient and render to you and your family the highest quality medical care.
We provide a full range of medical care from comprehensive examinations, screening tests, preventive care, urgent care, and pediatric services. Our facility is equipped with laboratory and radiology services so you can receive most preventive services without having to go anywhere else.
Our business office files claims to most insurance carriers. Our conscientious staff strives to keep accurate billing services for you. In order for our staff to do a good job, they need detailed information. We ask that you take the time to fill out the appropriate New Patient forms and bring them with you to your appointment.
LCFP Financial Policy requires payment at the time of service. If your insurance plan includes a co-payment, it is due at the time of service. We will file most insurance, but you are responsible for any balances. If you are without health insurance, our staff will require payment for the office call at the time of service. If, after 30 days from the date of service your account is not paid in full, all charges are your responsibility. Bring your insurance card to all visits and let us know of any changes. All new patients are required to read and sign the LCFP Financial Policy form. |
